The form below is for parents to complete if their child(ren) need a device (i-Pad).  Please keep in mind, we keep track of all of our devices through their serial numbers.  If a student received an i-Pad last year, they are expected to use that device again this year.

The form also has a section for completion in the event you have not yet received your OnCourse username and password.
